Case Coordination / Support
Case Managers visit clients in their homes for a confidential assessment of the challenges they face with living independently. Together, the client and Case Manager develop a personal care plan.
Hours of Service in 2021
Licensed Social Workers meet one on one with each senior in their home to discuss available services, coordinate their service, and maintain periodic contact.
Case coordination organizes access to services for persons 60 and older according to their level of need. It includes a comprehensive assessment and subsequent reassessments of personals aged 60 and over with the subsequent development and monitoring of service plans tailored to the individual and their needs.
Case coordinators broker existing community services while working to enhance informal support systems when feasible.
In addition, case coordinators identify and communicate appropriate community agencies to arrange for services and evaluate the effectiveness and benefit of services provided.
